The Truffle is a Hardmode NPC vendor who sells various mushroom-themed items including the AutoHammer which is needed to progress in-game. To get the Truffle to move in, there must be a vacant House in (or near) a Surface Glowing Mushroom biome (i.e. one that is above 0 elevation).
How do you summon the truffle toad?
The Truffle Toad doesn’t spawn on her own, and must be summoned using a Toadstool in the Glowing Mushroom biome.
What are the requirements for a truffle worm to spawn?
The Truffle Worm is a rare critter that spawns in the underground Glowing Mushroom biome during Hardmode. It is solely used to summon the Duke Fishron boss. In order to use it, it must be caught (e.g. with a Bug Net) and used as bait with a fishing pole in an Ocean biome.

Why is Truffle not spawning?
When attempting to spawn the Truffle, try to make sure enough Houses exist for other NPCs that have not yet spawned. Having too much Corruption or Crimson beneath the house may invalidate the housing. For the Truffle to move in, the house must be in a Surface Glowing Mushroom biome.
Why is Truffle not moving in?
Truffle will only move in during Hardmode, in a surface-area Mushroom biome (The moment that the music theme changes, the background darkens a bit and large mushrooms appears on the background.) then he will move in.
